Overall, the effects on weight loss of synthetic ephedrine plus caffeine and ephedra plus herbs containing caffeine were equivalent: weight loss of approximately 2 pounds/month more than was associated with placebo for up to 4 or 6 months of use. No studies assessed the long-term effects on weight loss; the longest published follow-up was 6 months.

  • Ephedrine vs. placebo: 5 studies. Ephedrine was associated with a statistically significant weight loss of 1.3 pounds/month more than was associated with placebo for up to 4 months of use.
  • Ephedrine plus caffeine vs. placebo: 12 studies. Ephedrine plus caffeine was associated with a statistically significant weight loss of 2.2 pounds/month more than was associated with placebo for up to 4 months of use.
  • Ephedrine plus caffeine vs. ephedrine hcl : 3 studies. Ephedrine plus caffeine was associated with a statistically significant weight loss of 0.8 pounds/month more than was associated with ephedrine alone.
  • Ephedra plus herbs containing caffeine vs. placebo: 4 studies. Ephedra plus herbs containing caffeine was associated with a statistically significant weight loss of 2.1 pounds/month more than was associated with placebo for up to 4 months of use.

History of Ephedra and Ephedrine

Ephedra is the name of a group of shrubs from the Ephedraceae family. There are many types of Ephedra plants, found from the United States to Europe and Asia. Some of the more common types of Ephedra plants are distachya Ma-huang), nevadensis (Mormon Tea) Sinica, Vulgaris and Equisetina.

When these plants grow, part of their metabolic process produces alkaloids. These alkaloids are used medicinally when the plant is dried. Some species of Ephedra plant have no alkaloid content in them at all. Others, such as Sinica, have an alkaloid content of 1-3% by dry weight.

The main alkaloids are ephedrine and pseudoephedrine. The concentration of these alkaloids depends on what species of ephedra is used. Certain types of Ephedra plants have more alkaloids than others. The concentration even in each species can be different; depending on soil and growing conditions.

Ephedra has been used for a very long time. It is mentioned in Chinese herbology texts from 2000 years ago. Nayagoshi Nagai, a chemist from Japan, was first able to isolate Ephedrine from the species Ephedra distachya. In China, traditionally Ephedra Ma-huang was used to treat those with the flu or lung problems, as well as treating bronchitis and colds. Today, China is the world’s largest producer of Ephedra.
